HOMA-IR Calculator

The HOMA-IR (Homeostatic Model Assessment for Insulin Resistance) approximates insulin resistance using fasting insulin and fasting glucose.

It is a simple, noninvasive estimate after an overnight fast. Direct methods such as the euglycemic clamp are more definitive but are resource-intensive; HOMA-IR is widely used in research and as an adjunct in clinical discussion.

Formula

HOMA-IR = (Fasting insulin in μIU/mL) × (Fasting glucose in mg/dL) ÷ 405

When you enter SI units (pmol/L for insulin, mmol/L for glucose), values are converted using:

• Insulin: pmol/L = μIU/mL × 6
• Glucose: mmol/L = mg/dL × 0.0555

Interpreting results

There is no single universally accepted cutoff. Common references include:

• Values between about 2.0 and 3.0 in many U.S. clinical and research settings
• ≥2.5 often used (for example in NHANES analyses) to indicate insulin resistance
• Lower cutoffs (about 1.4–2.5) are sometimes discussed for Asian populations

HOMA-IR is not a diagnosis and should not replace clinical judgment. Discuss results with your healthcare provider.

1. Use laboratory values from a fasting sample (about 8 hours overnight).
2. Choose units: μIU/mL + mg/dL or pmol/L + mmol/L.

Not reliable for: people with established diabetes on exogenous insulin, acute illness or stress, and some other groups (for example older adults or lean individuals in certain contexts). Results are assay- and population-dependent.
