After you eat carbohydrates, they are broken down into glucose. As blood glucose rises, the pancreas releases insulin. Insulin then acts like a signal that allows cells, particularly muscle and fat cells, to take up glucose.
With insulin resistance, cells do not respond to insulin as effectively as they should. To compensate, the pancreas may produce more insulin in an attempt to keep blood glucose within a healthy range. This can result in higher-than-normal insulin levels, sometimes for years before blood glucose becomes high enough to meet the criteria for prediabetes or type 2 diabetes.

The relationship between insulin resistance and weight is complex and works in both directions. Excess body fat, particularly fat stored around the abdominal organs, is strongly associated with insulin resistance. At the same time, metabolic changes associated with insulin resistance can make weight management more difficult.
Insulin also plays an important role in how the body stores and uses energy. When insulin levels are elevated, the body receives signals that favor energy storage. However, insulin is only one part of weight regulation. Appetite, calorie intake, physical activity, sleep, genetics, medications, hormones, and other health conditions can all influence body weight.
This is why insulin resistance should not be viewed as a simple explanation that a person is "unable to burn fat." Weight regulation is much more complicated than the action of a single hormone.
Why Can Weight Loss Be More Difficult with Insulin Resistance?
Several factors may contribute to difficulty losing weight when insulin resistance is present.
Higher Insulin Levels
During the early stages of insulin resistance, the pancreas often compensates by producing more insulin. Insulin promotes glucose uptake and energy storage and reduces the breakdown of stored fat. Persistently elevated insulin levels may therefore be one factor affecting how the body manages stored energy.
This does not prevent weight loss when the body is consistently using more energy than it receives, but it illustrates why metabolic health and weight regulation are closely connected.
Increased Hunger and Appetite
Some people with insulin resistance experience increased hunger or difficulty feeling satisfied after meals. Blood glucose fluctuations and the body's altered metabolic response can contribute to appetite changes.
Highly refined carbohydrates and sugary foods may also cause rapid changes in blood glucose in some people. Choosing meals containing protein, fiber, vegetables, whole grains, legumes, nuts, and other minimally processed foods can often improve satiety and make a reduced-calorie eating pattern easier to maintain.
Excess Abdominal Fat
Visceral fat is the fat stored around internal organs in the abdomen. Higher amounts of visceral fat are strongly associated with insulin resistance and metabolic disease.

As excess body fat decreases, particularly visceral fat, cells can become more responsive to insulin. This means the body may require less insulin to manage glucose effectively.
Importantly, very large amounts of weight loss are not always necessary before metabolic improvements occur. Current diabetes guidelines recognize that losing approximately 5% to 7% of initial body weight can improve blood glucose and other cardiovascular risk factors in people with diabetes and overweight or obesity. Greater sustained weight loss may provide additional metabolic benefits and, in some people with type 2 diabetes, may contribute to diabetes remission.
For example, a person weighing 100 kg would reach a 5% weight-loss target after losing 5 kg. Although this may seem modest compared with some commercial weight-loss goals, it can still have important health benefits.
What Is the Best Way to Lose Weight with Insulin Resistance?
There is no single diet that is appropriate for everyone with insulin resistance. A sustainable approach usually combines nutrition, regular physical activity, adequate sleep, behavioral changes, and appropriate medical treatment when needed.
Choose Foods That Support Blood Sugar Control
A balanced eating pattern should emphasize nutrient-dense foods and appropriate portions rather than eliminating entire food groups unnecessarily.
Useful choices include:
- Non-starchy vegetables
- Whole fruits
- Beans, lentils, and other legumes
- Whole grains in appropriate portions
- Eggs, fish, poultry, tofu, and other protein sources
- Nuts and seeds
- Unsweetened or minimally processed dairy products when appropriate
It is generally helpful to reduce frequent consumption of sugar-sweetened beverages, sweets, refined grains, and heavily processed foods. Portion size and total calorie intake remain important because weight loss ultimately requires the body to use stored energy over time.
Include Enough Protein and Fiber
Protein and fiber can help increase fullness after meals. Higher-fiber foods also tend to have a more gradual effect on blood glucose compared with many highly refined carbohydrates.
Rather than eating carbohydrates alone, combining them with protein, vegetables, healthy fats, or fiber-rich foods can create more balanced meals.
Exercise Regularly
Physical activity is particularly valuable for people with insulin resistance because working muscles can use glucose more effectively. Regular exercise can improve insulin sensitivity even before substantial weight loss occurs.
A combination of aerobic activity and resistance training is generally beneficial. Walking, cycling, swimming, and similar activities can improve cardiovascular fitness, while resistance exercises help maintain or increase muscle mass.
For many adults, a goal of at least 150 minutes of moderate-intensity physical activity per week is recommended, although the appropriate amount depends on a person's health, fitness level, medications, and diabetes-related complications.
Pay Attention to Sleep
Sleep is sometimes overlooked during weight-loss efforts. Inadequate or irregular sleep can affect appetite, food choices, glucose regulation, and insulin sensitivity.
Improving sleep habits may therefore support both metabolic health and long-term weight management.
Can Medications Help?
Medication may be appropriate for some people, but treatment depends on the individual's medical history, weight, blood glucose levels, cardiovascular risk, and other health conditions.
Metformin is commonly used to treat type 2 diabetes and is sometimes prescribed for people at particularly high risk of developing diabetes. Other medications used for diabetes or obesity can also produce significant weight loss while improving glucose control.
For example, GLP-1 receptor agonists and related medications can reduce appetite, improve blood glucose control, and promote weight loss in appropriately selected patients. These medications are not necessary or suitable for everyone, and they should be prescribed and monitored by a qualified healthcare professional.
Can Insulin Resistance Be Reversed?
Insulin sensitivity can improve substantially. In some people, lifestyle changes, weight loss, physical activity, and appropriate medical treatment can bring blood glucose levels back into a healthier range and significantly reduce insulin resistance.
However, the term "reversed" should be used carefully. A person's underlying tendency toward insulin resistance may remain, particularly when genetic or metabolic risk factors are present. Returning to previous lifestyle patterns or regaining substantial weight may cause insulin resistance to worsen again.
For this reason, the goal is not simply short-term weight loss. Long-term improvements in nutrition, physical activity, sleep, body composition, and metabolic health are more important.
Weight Loss Is Still Possible with Insulin Resistance
Insulin resistance can make weight management more complicated, but it does not make weight loss impossible. The same biological relationship that connects excess body fat with insulin resistance also provides an opportunity for improvement. Losing excess weight can improve insulin sensitivity, and improving metabolic health can make long-term weight management easier.
If weight loss remains difficult despite consistent lifestyle changes, it is worth discussing the issue with a healthcare professional. Diabetes, medications, sleep disorders, PCOS, thyroid disease, and other factors may need to be considered. Depending on the individual, structured nutrition support, diabetes treatment, weight-management medications, or other medical interventions may also be appropriate.
The most effective approach is one that can be maintained over time and addresses both weight and metabolic health rather than focusing on the number on the scale alone.
